"West Ham United will bid an emotional farewell to the Boleyn Ground with a Barclays Premier League fixture against Swansea City. The Swans will provide the final league opposition in E13 on 7 May, prior to the Club’s exciting move to the new Stadium on Queen Elizabeth Olympic Park."
